If that includes you, the next step is scheduling an appointment to establish care with a new doctor in the new year. Starting over can feel overwhelming, however, especially if you\u2019ve been with the same doctor for a long time. Leaving a familiar relationship and having to retell your medical story from scratch isn\u2019t easy.<img loading=\"lazy\" decoding=\"async\" class=\" wp-image-4928 alignleft\" src=\"http:\/\/peykmagazine.com\/en\/wp-content\/uploads\/2026\/01\/Majd.jpg\" alt=\"\" width=\"375\" height=\"212\" \/><\/p>\n<p class=\"p1\">The good news? Meeting a new doctor doesn\u2019t have to be stressful. With a little preparation ahead of time, your first visit can be smooth, productive, and even reassuring. Knowing what your doctor will focus on and what information they\u2019ll be looking for can help you make the most of your appointment.<\/p>\n<p class=\"p1\">Below is a guide to what your new doctor will want to know and what you should be ready to share to ensure a seamless transition of care. Grab a blank sheet of paper and start <b>typing or writing out the outline below<\/b>, including the relevant details for each section. Use <b>bullet points<\/b> to keep everything clear, organized, and easy to review. Once completed, <b>print it out and bring it with you<\/b> to your first visit with your new doctor.<\/p>\n<h4 class=\"p2\"><span style=\"color: #800000;\"><b>Medication List<\/b><\/span><\/h4>\n<p class=\"p1\">If you\u2019re taking just one or two medications or you know your medications well, you can simply bring a written list with you to your appointment. Be sure the list includes the <b>full name <\/b>of each medication exactly as it appears on the bottle, along with the <b>dose<\/b> and <b>how many times per day <\/b>you take it.<\/p>\n<p class=\"p1\">Including the full name matters more than many people realize. Some medications have similar names, but work differently depending on the formulation. For example, metoprolol <i>succinate<\/i> is different from metoprolol <i>tartrate<\/i>\u2014one is long-acting while the other is short-acting. Likewise, bupropion <i>SR<\/i> and bupropion <i>XL<\/i> are not interchangeable and are prescribed differently.<\/p>\n<p class=\"p1\">If you take several medications\u2014or if remembering names, spelling, or pronunciation is challenging\u2014bringing your <b>actual medication bottles<\/b> to your first visit is extremely helpful (and often worthwhile for ongoing visits as well). As a physician, I much prefer reviewing the bottles firsthand, especially when meeting a patient for the first time. This allows me to catch potential errors, clarify dosing, and confirm that medications listed in the chart are truly being taken\u2014and taken correctly. It\u2019s not uncommon for discrepancies to exist, and reviewing bottles helps eliminate confusion and misunderstandings.<\/p>\n<p class=\"p1\">Lastly, don\u2019t forget to include or bring <b>over-the-counter medications<\/b>, <b>topical creams<\/b>, <b>vitamins<\/b>, <b>supplements<\/b>, <b>insulin<\/b>, <b>injectables<\/b>, and <b>inhalers<\/b>. These are often overlooked, but they are an important part of your overall treatment plan and medical care.<\/p>\n<h4 class=\"p2\"><span style=\"color: #800000;\"><b>Chronic Diagnoses List<\/b><\/span><\/h4>\n<p class=\"p3\">Most doctors today use electronic health records (EHRs). If your previous doctor offered a patient portal, this step may be very straightforward. Simply log in to your former doctor\u2019s portal and look for your list of medical diagnoses, often labeled as a <b>\u201cproblem list\u201d<\/b> or <b>\u201cpast medical history.\u201d<\/b> Printing this list and bringing it to your first visit can be extremely helpful.<\/p>\n<p class=\"p1\">If you don\u2019t have portal access, don\u2019t worry. Just write down the <b>major chronic conditions<\/b> you\u2019ve been diagnosed with. Your current medications can serve as helpful reminders\u2014<i>\u201cLisinopril\u2026 right, that\u2019s for my high blood pressure,\u201d<\/i> for example.<\/p>\n<p class=\"p4\">Common chronic conditions include:<\/p>\n<p class=\"p6\"><b>\u2022 <\/b>Diabetes<\/p>\n<p class=\"p6\"><b>\u2022 <\/b>High blood pressure<\/p>\n<p class=\"p6\"><b>\u2022 <\/b>High cholesterol<\/p>\n<p class=\"p6\"><b>\u2022 <\/b>Heart disease<\/p>\n<p class=\"p6\"><b>\u2022 <\/b>Chronic obstructive pulmonary disease (COPD)<\/p>\n<p class=\"p6\"><b>\u2022 <\/b>Asthma<\/p>\n<p class=\"p6\"><b>\u2022 <\/b>Hypothyroidism<\/p>\n<p class=\"p6\"><b>\u2022 <\/b>Seasonal or environmental allergies<\/p>\n<p class=\"p6\"><b>\u2022 <\/b>Rheumatoid arthritis<\/p>\n<p class=\"p1\">These are just examples. Focus on <b>ongoing, long-term conditions<\/b> that require monitoring or treatment. There\u2019s no need to list temporary issues like a brief episode of back pain a couple of years ago or the occasional cold. Your new doctor is primarily interested in your current, chronic health conditions so they can best manage your care moving forward.<\/p>\n<h4 class=\"p2\"><span style=\"color: #800000;\"><b>Surgeries\/Procedure List<\/b><\/span><\/h4>\n<p class=\"p1\">You may not remember every medical event in detail; wisdom tooth extractions, for example, aren\u2019t always that memorable. But if you\u2019ve had an organ removed, a joint replaced, or a medical device implanted, those are details your doctor absolutely needs to know.<\/p>\n<p class=\"p1\">Think about your body <b>from head to toe<\/b> and make a list of any surgeries or procedures you\u2019ve had. An exact date isn\u2019t always necessary, especially for procedures done many years ago, but including an <b>approximate year<\/b> is very helpful.<\/p>\n<p class=\"p4\">Below are examples of common surgeries and procedures to include:<\/p>\n<p class=\"p6\"><b>\u2022 Cholecystectomy<\/b> (gallbladder removal)<\/p>\n<p class=\"p6\"><b>\u2022 Appendectomy<\/b> (appendix removal)<\/p>\n<p class=\"p6\"><b>\u2022 Tonsillectomy<\/b> (tonsil removal)<\/p>\n<p class=\"p7\"><b>\u2022 Hysterectomy<\/b> (uterus removal; please note whether ovaries and\/or cervix were also removed)<\/p>\n<p class=\"p6\"><b>\u2022 Hernia repair<\/b> (specify location: groin, belly button, or abdominal wall)<\/p>\n<p class=\"p6\"><b>\u2022 Knee arthroscopy<\/b><\/p>\n<p class=\"p6\"><b>\u2022 Knee joint replacement<\/b><\/p>\n<p class=\"p6\"><b>\u2022 Rotator cuff repair<\/b> (shoulder)<\/p>\n<p class=\"p6\"><b>\u2022 Spinal fusion<\/b> (cervical, thoracic, or lumbar spine)<\/p>\n<p class=\"p6\"><b>\u2022 Skin cancer removal<\/b> (note body location and type of skin cancer)<\/p>\n<p class=\"p6\"><b>\u2022 Breast lumpectomy<\/b> (include final diagnosis after biopsy)<\/p>\n<p class=\"p6\"><b>\u2022 Mastectomy<\/b> (include final diagnosis after biopsy)<\/p>\n<p class=\"p6\"><b>\u2022 Breast biopsy<\/b> (include final diagnosis after biopsy)<\/p>\n<p class=\"p6\"><b>\u2022 Fracture repairs<\/b> (specify which bone)<\/p>\n<p class=\"p6\"><b>\u2022 Cataract surgery<\/b><\/p>\n<p class=\"p6\"><b>\u2022 Pacemakers<\/b><\/p>\n<p class=\"p1\">Be sure to indicate <b>which side of the body, <\/b>left or right, was involved for each procedure. These details help your doctor better understand your medical history and safely guide your care.<\/p>\n<h4 class=\"p2\"><span style=\"color: #800000;\"><b>List of Specialists<\/b><\/span><\/h4>\n<p class=\"p1\">Your primary care doctor serves as the central hub for your healthcare. That means they should be kept informed about your overall health, including care you receive from specialists. One of the best ways to ensure everyone stays aligned is to ask your specialist\u2019s office to <b>forward their visit notes<\/b> to your primary care doctor.<\/p>\n<p class=\"p1\">In a perfect world, this would happen automatically, but in reality, it doesn\u2019t always. For that reason, bringing a <b>list of your specialists<\/b>, along with their <b>names and phone numbers<\/b>, to your first visit is extremely helpful. This allows your primary care provider to reach out directly when needed and better coordinate your care.<\/p>\n<p class=\"p1\">Keeping this information handy at your first visit helps create smoother communication and more comprehensive, connected care for you.<\/p>\n<h4 class=\"p2\"><span style=\"color: #800000;\"><b>Social History<\/b><\/span><\/h4>\n<p class=\"p1\">Most primary care doctors will also review your social history, as it provides important context for your overall health. This typically includes:<\/p>\n<p class=\"p6\"><b>\u2022 Occupation<\/b><\/p>\n<p class=\"p6\"><b>\u2022 Marital status<\/b> and <b>number of children<\/b><b><\/b><\/p>\n<p class=\"p6\"><b>\u2022 Tobacco use<\/b>, including whether you currently smoke or have smoked in the past<\/p>\n<p class=\"p8\"><b>\u2022 <\/b>Number of packs per day<\/p>\n<p class=\"p8\"><b>\u2022 <\/b>Total number of years smoked<\/p>\n<p class=\"p8\"><b>\u2022 <\/b>Year you quit (if applicable)<\/p>\n<p class=\"p6\"><b>\u2022 Alcohol use<\/b>, usually measured as the number of servings per week<\/p>\n<p class=\"p6\"><b>\u2022 Drug use history<\/b>, which may include marijuana use (not uncommon). A history of <b>intravenous drug use<\/b> and\/or <b>substance dependence or addiction<\/b> is especially important to share, as it can significantly impact medical care and decision-making.<\/p>\n<p class=\"p1\">Providing this information honestly helps your doctor better assess health risks, recommend appropriate screenings, and deliver safer, more personalized care.<\/p>\n<h4 class=\"p2\"><span style=\"color: #800000;\"><b>Family History<\/b><\/span><\/h4>\n<p class=\"p9\">Write down each of your <b>first-degree relatives<\/b> and any medical conditions they have been diagnosed with. A first-degree family <span class=\"s1\">history is the most important to share and includes <b>parents, siblings, <\/b><\/span><b>and children<\/b>. While conditions affecting cousins, aunts, and uncles can sometimes provide helpful background, they are not considered as strong or direct a medical link.<\/p>\n<p class=\"p1\">When gathering information from your family members, focus on <b>chronic and hereditary conditions<\/b>, such as:<\/p>\n<p class=\"p6\"><b>\u2022 <\/b>Diabetes<\/p>\n<p class=\"p6\"><b>\u2022 <\/b>High blood pressure (hypertension)<\/p>\n<p class=\"p6\"><b>\u2022 <\/b>High cholesterol<\/p>\n<p class=\"p6\"><b>\u2022 <\/b>Heart disease<\/p>\n<p class=\"p6\"><b>\u2022 <\/b>Heart attacks<\/p>\n<p class=\"p6\"><b>\u2022 <\/b>Stroke<\/p>\n<p class=\"p6\"><b>\u2022 <\/b>Cancer<\/p>\n<p class=\"p1\">Sharing an accurate family health history helps your doctor better assess inherited risks, recommend appropriate screenings, and tailor preventive care to you.<\/p>\n<h4 class=\"p2\"><span style=\"color: #800000;\"><b>Allergy List<\/b><\/span><\/h4>\n<p class=\"p1\">It\u2019s important to share any prior reactions you\u2019ve had to medications or foods with your doctor. Be sure to include <b>what the reaction <\/b>was, not just the name of the medication or food.<\/p>\n<p class=\"p1\">Many reactions are actually <b>intolerances<\/b> rather than true allergies, such as nausea, diarrhea, or abdominal discomfort. These should still be listed, as they help guide future treatment decisions.<\/p>\n<p class=\"p4\"><b>True allergic reactions<\/b> may include:<\/p>\n<p class=\"p6\"><b>\u2022 <\/b>Rash or hives<\/p>\n<p class=\"p6\"><b>\u2022 <\/b>Lip or facial swelling<\/p>\n<p class=\"p6\"><b>\u2022 <\/b>Throat swelling or difficulty breathing<\/p>\n<p class=\"p1\">If you\u2019ve ever experienced a severe allergic reaction, such as <b>anaphylaxis<\/b> or <b>angioedema<\/b>, be sure to note this and indicate whether you carry an <b>EpiPen<\/b>.<\/p>\n<p class=\"p1\">It\u2019s also helpful to describe <b>how severe<\/b> the reaction was. For example, mild diarrhea after taking an antibiotic may not prevent you from using it again. In contrast, developing <i>C. difficile colitis<\/i>\u2014a potentially serious infection causing severe diarrhea\u2014would make your doctor much more cautious about prescribing that medication in the future.<\/p>\n<h4 class=\"p2\"><span style=\"color: #800000;\"><b>Important Studies<\/b><\/span><\/h4>\n<p class=\"p1\">You do <i>not<\/i> need to transfer hundreds or thousands of pages of old medical records when establishing care with a new doctor. In many cases, that\u2019s unnecessary and inefficient. For most patients, providing copies of the following is sufficient:<\/p>\n<p class=\"p6\"><b>\u2022 Most recent colonoscopy<\/b>, including any pathology reports and recommended follow-up interval<\/p>\n<p class=\"p6\"><b>\u2022 Last Pap smear<\/b><\/p>\n<p class=\"p6\"><b>\u2022 Last mammogram<\/b><\/p>\n<p class=\"p6\"><b>\u2022 Last DEXA<\/b> (bone density scan)<\/p>\n<p class=\"p6\"><b>\u2022 Any prior CT scans <\/b>or<b> MRIs<\/b><\/p>\n<p class=\"p6\"><b>\u2022 Recent laboratory results<\/b> (within the past year, if available)<\/p>\n<p class=\"p10\">Some patient portals will allow you access to the above; you can simply print and bring to your visit if so. Older lab work and extensive records are often not needed initially. Your doctor\u2019s office can always request additional records later if they become relevant.<\/p>\n<p class=\"p4\">Other Clinical Information<\/p>\n<p class=\"p1\">When establishing care with a new primary care doctor, expect some additional intake information and paperwork:<\/p>\n<p class=\"p6\"><b>\u2022 Intake forms:<\/b> These may be electronic or paper. Calling the office ahead of time to ask if you can complete them in advance can save time and allow you to fill them out at your own pace.<\/p>\n<p class=\"p6\"><b>\u2022 Insurance card:<\/b> Some offices may request insurance information ahead of time, but it\u2019s always wise to bring your card to every visit.<\/p>\n<p class=\"p6\"><b>\u2022 Pharmacy information:<\/b> Be prepared to provide the name, address, and phone number of your preferred pharmacy.<\/p>\n<h4 class=\"p2\"><span style=\"color: #800000;\"><b>In Summary<\/b><\/span><\/h4>\n<p class=\"p1\">Many medical offices that use electronic health records with patient portals offer a <b>summary section<\/b> that includes some\u2014but not all\u2014of the information above. This may be labeled \u201cSynopsis,\u201d \u201cFace Sheet,\u201d \u201cPatient Summary,\u201d or something similar. Accessing your portal and printing this information can save you some time and effort.<\/p>\n<p class=\"p3\">If portal access isn\u2019t available, consider keeping an up-to-date<b> written or typed list<\/b> for your own records. This can be invaluable not only for your first visit with a new primary care doctor, but also for future specialist visits and for your own personal health reference.<\/p>\n<p class=\"p1\">Now that you know what your doctor will be looking for at your first appointment, you\u2019ll be well prepared. Be sure to schedule your initial visit specifically to <b>establish care<\/b>, as there\u2019s often a lot of important information to review.
